Information for Applicants

The following information should be read carefully.

  1. Application Form
    • To become a Member of PharMIG, each applicant needs to complete an official 'Application Form for Membership' which is obtainable via the Company Secretary or the web site.
    • Corporate applicants may nominate up to four individuals on the application form as PharMIG contacts. All levels of Corporate Membership are entitled to send any other Company employee to PharMIG events other than the four specified on the form at the Member price.
    • For Academic/Student level, a single name must be entered on the form and it is only this individual that may attend PharMIG events at the Member price. Any Hospital Pharmacy facility may join on an individual basis or as a Corporate Member submitting the appropriate fee.
    • Each form has a 'Declaration' section that binds Members to comply with the Terms of Reference of the Group, which in turn, are governed by the Memorandum and Articles of Association of the Company. Failure to do so can lead to termination of Membership.
  2. Annual Subscription Fees
    • The current annual subscription is £360 for all levels of Corporate Membership except the Academic/Student level, which is £60. The annual subscription is not refundable and there are no discounts for joining mid year etc. There is no VAT charged, PharMIG is not a VAT registered Company.
  3. Payment Details
    • The preferred method of payment is by cheque. Alternatively, a direct money transfer to the PharMIG bank account can be made; details will be supplied upon request.
    • For all applicants, whether they are current Members or new applicants, an invoice will automatically be sent once a completed application form has been received i.e. an invoice will be supplied only upon request. PharMIG regrets that payment by Visa, Amex or Mastercard is not possible.
  4. Acknowledgement/Receipt
    • All applications received will be acknowledged as soon as possible. If a receipt is required for any payment, please indicate this in a separate covering note.
  5. Change of Details
    • Please notify any changes to those registered for Membership to the Company Secretary, Poly Hajipieris. It is recommended that you keep a copy of your application form for reference.
